During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 10/20/2025, as a part of the licensure period. The on-site inspection began at 10:00am and ended at 11:00am. The inspector reviewed compliance in the areas listed above. There were 3 children present and 2 caregivers; the point total was 13. The inspector reviewed 3 children?s records and 2 caregiver/household members records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver a current Report of Tuberculosis Screening form every two years.

The TB report for caregiver #1 is overdue by six months.
Plan of Correction: Provider will have TB screening completed by 10/25/2025.
Standard 8VAC20-800-780-B
Annually, the provider shall review with the parent the emergency contact information and obtain the parent's signed acknowledgment of the review.

There was not documentation that provider had reviewed emergency contact information with parent of a child who has been attending for 18 months.
Plan of Correction: Provider will review information with child's parent today 10/20/2025.
Standard 8VAC20-800-80-B
The provider shall verify the identity and age of a child by viewing approved document within seven business days of child's first day of attendance.

Provider did not view approved documentation until four months after first day of attendance.
Plan of Correction: Provider will ensure verification of identity is obtained within 7 days of attendance.
