Inspector Notes
The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department regarding allegations related to staff qualifications and training, administrative and staffing and supervison. The inspection was initiated on April 18, 2023. During the investigation, interview with staff was completed and observations were made in the center, and documentation was reviewed.
There was a preponderance of the evidence gathered during the investigation support the allegation in regards to administrative and staff training; therefore, the program allegation is determined to be valid.
These violations are listed on the violation notice issued to the facility.
Violations
2Evidence: During the inspection in the toddler classroom, staff #1 and staff #2 were the only staff in the classroom . Both staff did not have CPR and First aid.
Confirmed during the inspection with staff in charge.
Evidence:
Incident report and staff confirmed there was incident on February 28th, 2023 and 911 was called after child #1 fell backwards and hit the back of her head. The center did not report the outside medical attention with in two buisness day. During the inspection, on April 18,2023 inspection confirmed center contacted 911 for medical assistance.
