Inspector Notes

This renewal inspection was initiated by licensing staff and concluded on 10/14/2022, between the hours of 10 am to 12 pm. There were 2 preschool-age children present (4 points) with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 2 children records, 1 caregiver record, and 3 staff records were reviewed.

Information gathered during the inspection determined that there were (3) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Based on review, 2 of 2 children files indicated that not all of the required information was contained within each child's record.

Evidence -
1. On the date of inspection (10/14/2022), the action for the allergy listed was not available for review for the following children: Child #1, and Child #2.
2. The need of the action for allergy listed was previously cited for Child #1 during a monitoring inspection conducted on 09/28/2022.

Based on review, 1 of 2 children files indicated that the verification of each child's proof of identification was not on file.

Evidence -
1. On the date of inspection (10/14/2022), the proof of identification was not available for Child #2.
2. The start date listed for Child #2 was listed as 10/01/2022. Child #2 was observed on site on the date of inspection.

Based on review, 1 of 2 children files indicated that the Provider did not obtain documentation that a child had been adequately immunized (according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations) before attending the Family Day Home (FDH).
