Inspector Notes

An unannounced non mandated inspection was conducted today from 12:05pm to 1:10pm. There were 4 children (10 points) in care of 1 caregiver. The children were observed playing outdoors, playing with blocks and puzzles. Diapering was observed. 4 children's records and 1 family day home document, emergency drill records and attendance record was reviewed. There were no children with medications or diagnosed food allergies on site.

Standard 8VAC20-800-100-A
Based on records review, the provider did not obtain documentation of a physical exam for children in care within 30 days of enrollment.
Evidence:
1. First day of enrollment for the children enrolled are as follows: Child # 1: 8/15/22; Child # 2: 1/1/22; Child # 3: 8/2021 and Child # 4: ?.
2. Documentation of a physical exam was not obtained for any of the four children.
Plan of Correction: The forms will be obtained.
Standard 8VAC20-800-60-B
Based on records review, information in children's records was not complete.
Evidence: Child # 1, Child # 2, Child # 3 and Child # 4's record was missing the medical insurance information and the policy number.
Plan of Correction: All the missing information will obtained from the parents.
Standard 8VAC20-800-680-B
Based on observation, the caregiver did not ensure that children's hands were washed after diapering.
Evidence: The provider changed Caregiver # 1'sdiaper, but did not wash Child # 1's hands after the diapering was completed.
Plan of Correction: I will ensure that children's hands are washed after diapering.
Standard 8VAC20-800-690-D
Based on observation, the diapering surface was not cleaned after being used.
Evidence: The provider changed Child # 1's diaper on the diaper changing pad. The provider did not use soap and water to clean the pad, prior to using the disinfectant solution.
Plan of Correction: I will follow the two step cleaning and disinfecting process.
Standard 8VAC20-800-70-A
Based on records review, children's files were not complete.
Evidence:
1. Child # 1, Child # 2, Child # 3 and Child # 4's records were missing information related to the following: medication administration policy, liability insurance policy, and provisions of the family day home's emergency preparedness plan.
Plan of Correction: The missing documents will be completed and filed in each child's record.
