Inspector Notes

An unannounced renewal inspection was conducted on 11/02/2022. The inspector was on site at the family day home from approximately 11:17am to 1:00pm. There were 4 children in attendance and a total of 1 caregiver present. Interviews were held with the caregiver throughout the inspection, and the inspector interacted with children when appropriate. The yard and all areas of the home were inspected today. One caregiver/household member record and four children?s records were reviewed during this inspection. There were two violations found during today?s inspection. See the violation notice for more details.

Standard 8VAC20-800-770
Based on observation, the provider did not ensure a working portable battery-operated weather band radio, and extra batteries were kept in a designated area and be available to caregivers at all times.
Evidence:
The provider did not have enough batteries for the battery-operated radio and therefore the radio was not able to work as required.
Plan of Correction: Will buy more batteries today.
Standard 8VAC20-800-980-A
Based on record review, the provider the not ensure three of five children?s records contained documentation of general written permission from parents for the provider to take the child off the premises of the family day home.
Evidence:
1. The record of child #1 (enrollment date: 5/16/22) did not have documentation of written permission to be taken of the premises of the family day home from the child?s parent. The provider stated that the children are taken to the park regularly.
2. The record of child #2 (enrollment date: 7/25/22) did not have documentation of written permission to be taken of the premises of the family day home from the child?s parent. The provider stated that the children are taken to the park regularly.
3. The record of child #4 (enrollment date: 8/23/21) did not have documentation of written permission to be taken of the premises of the family day home from the child?s parent. The provider stated that the children are taken to the park regularly.
