During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, monitoring inspection was completed on October 3, 2025. The inspection began at 12:00 PM and ended at 2:15 PM. The inspector reviewed compliance in the areas listed above. There were 21 children present and six staff. The inspector reviewed six children?s records and four staff records. This inspection included document review, tour of the facility, interviews, and observations.

The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/10/2025. A POC submitted after this date will not appear on the public website.

The center's procedures for administering medication is required to include duration of the parent's authorization for medication, provided that it shall expire or be renewed after 10 work days. Long-term prescription drug use and over-the- counter medication may be allowed with written authorization from the child's physician and parent.

Child A's records did not contain written authorization from the children's physician and parent for long-term prescription and over-the-counter medications.
Plan of Correction: The staff will request the written authorization from the parents and it will be placed in the children's records.
