During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, monitoring inspection was completed on October 17, 2025. The inspection began at 12:50 PM and ended at 1:50 PM. The inspector reviewed compliance in the areas listed above. There were three children present and one caregivers; the point total was eight. The inspector reviewed three children?s records and one caregiver records.This inspection included document review, tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on November 3, 2025. A POC submitted after this date will not appear on the public website.
Violations
1Child A's record was missing the work address and phone number for both parents, physician address and phone number, and the address for one emergency contact. The information was 4 months overdue. Child B's record was missing one parent's employer's name, work address, and phone number, physician's address and phone number, and medical policy information.
