Inspector Notes
A complaint inspection was initiated on November 9, 2022 and concluded with an on-site inspection with an outside agency on December 6, 2022. A complaint was received in the Office of Child Care Health & Safety (Valley) on November 3, 2022, regarding number of children in care, care of children and safety issues. The inspector reviewed compliance in areas of administration, care of children, physical equipment and environment. Documentation of sign-in and sign-out sheets for the month of October 2022 were reviewed, observations and an interview with the provider were conducted.
The information gathered during the investigation supports the allegation, so the complaint is determined to be valid. Please complete a "plan of correction to be taken", and the "date to be corrected" for each violation cited on the violation notice. Please return to the Licensing Inspector within five calendar days of receipt.

Violations
3Evidence:
1. The Family Day Home License effective 08/01/2022-01/31/2023 was issued for a capacity of five children in care at one time.
2. The sign-in/sign-out sheet dated 10/18/2022 indicated seven children in care for ten minutes, and six children in care for 4.5 hours.
3. The sign-in/sign-out sheet dated 10/20/2022 indicated six children in care for thirty minutes.
4. The sign-in/sign-out sheet dated 10/25/2022 indicated seven children in care for 38 minutes, and six children in care for an additional three minutes.
5. The sign-in/sign-out sheet dated 10/28/2022 indicated seven children in care for 16 minutes, and six children in care for 3 hours 22 minutes.
6. The provider verified during an interview, on 12/06/2022, the dates and number of children in care for the four days in October 2022.
Evidence:
1. On 12/06/2022 when the Inspector arrived at the residence the children were observed in the foyer area near the steps that lead up to the second level of the home. There was no gate at the bottom of the stairs.
2. The gate was observed to be leaning against the wall.
3. There were two children in care under the age of two years of age. One child was 13 months old and the other 19 months old.
Evidence:
1. The sign-in/sign-out sheet dated 10/18/2022 indicated a total of seven children in care for approximately 10 minutes for a total of 20 points, and six children in care for 20 minutes for a total of 17 points. There was only one caregiver present.
2. The sign-in/sign-out sheet dated 10/20/2022 indicated a total of seven children in care for approximately 30 minutes for a total of 18 points. There was only one caregiver present.
3. The sign-in/sign-out sheet dated 10/25/2022 indicated a total of seven children in care for approximately 38 minutes for a total of 20 points, and six children in care for an additional three minutes for a total of seventeen points. There was only one caregiver present.
4. The sign-in/sign-out sheet dated 10/28/2022 indicated a total of seven children in care for approximately 20 points, and six children in care for 3 hours and 22 minutes for a total of 17 points. There was only one caregiver present.
