Inspector Notes
An unannounced self-reported inspection was initiated on March 20, 2023, regarding concerns of care and protection and supervision of a child. The inspection was completed on April 3, 2023. Interviews were conducted and video footage was reviewed. Based on the evidence gathered, violations were cited. For violation details, please refer to the Violation Notice. Based on interviews and a review of electronic video footage, the center did not ensure that children under 10 years of age were always within actual sight and sound supervision of staff.
Evidence
On Friday, March 17, 2023, at approximately 7:59:24am, staff #1, #2, #3 and #4 were in the Toddler 2 classroom supervising 12 children.
13-month old Child #1 was observed exiting the Toddler 2 classroom at approximately 7:59:24am, without staff #1, #2, #3 and #4 noticing the child leaving the classroom.
Child #1 walked into the hallway, down the hall, passing the Toddler 1 and Infant 1 classrooms.
Child #1 was observed entering the Infant 2 classroom.
Child #1 began to touch the toys on the shelf in the classroom.
Staff #5 was observed looking into the Infant 2 classroom at 8:00:53am, observing child #1 alone in the classroom.
Staff #5 went back to Toddler 2 classroom to ask staff #1, #2, #3 and #4 why child #1 was alone in the Infant 2 classroom.
It was determined child #1 was not within actual sight and sound of a staff member for 1 minute and 30 seconds.

Plan of Correction: Staff #2, Staff #3, and Staff #4 each received a 3 day suspension without pay and Staff #4 was ultimately terminated at the end of the suspension period. Our policies have been updated to explicitly require that classroom doors be kept closed while children are present in the room. All staff have been notified in writing of this policy. Additionally, a member of the management team will be onsite during the opening and closing each day to ensure that standards are maintained.
