Based on review of video footage, an infant was not placed in the crib on their back.

On August 1, 2023 between 8:00am and 8:15am, staff #1 placed child #1 in their crib on their belly. Even when an infant is able to easily turn over from the back to the belly position and they are placed in the crib, they must still be put on their back, but allowed to adopt whatever position they prefer.

Plan of Correction: Staff thought since the child could turn over by themselves, they could lay the child down on their belly. Staff was immediately retrained on safe sleep practices.
