Inspector Notes

An investigation was initiated on May 23, 2024 regarding a complaint received by the Fairfax Licensing Office on May 20, 2024 alleging a breach of confidentiality and child behavioral concerns. Interviews were conducted, video footage, staff records and policies and procedures were reviewed. The information gathered during this inspection supports the allegations that the center was out of compliance with certain standards. The complaint is deemed valid. Based on interviews with staff, the operation responsibilities of the licensee did not ensure that the center's activities and services are maintained in compliance with the center's own policies and procedures that are required by the standards.
Evidence
Staff #1 and #2 confirmed during a parent teacher conference a center policy on confidentiality was breached by staff #2. Staff #2 discussed another child during the conference.
Plan of Correction: All staff will be retrained on the required confidentiality practices including what information must be kept confidential. This training will occur by June 21st.
Disciplinary action has been taken for Staff #2.
