An unannounced, on-site focused inspection was initiated on and completed on 5/19/2025. The Department entered into a Consent Agreement with the facility on April 15, 2025.
The on-site inspection began at 8:30AM and ended at 9:00AM. The inspector reviewed compliance in the areas listed above. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined no violations with applicable standards or law.
