An unannounced, on-site monitoring inspection was initiated on May 29, 2024 from 11:00 a.m. to 2:00 p.m. The inspector reviewed compliance in the areas listed above. There were 112 children present and 21 staff. The inspector reviewed 10 children?s records and 10 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
Out of state background checks shall be conducted as required. Staff 2, Staff 4, and Staff 10 indicated on their sworn disclosure statement s that they had lived outside the state of Virginia within the past five years. There was no documentation to support that two required out of state criminal record checks were conducted prior to employment. There was no documentation that three out of state searches of the child abuse and neglect registry were submitted within 30 days of employment as required.

Repeat violation. Documentation of a completed search of the central registry (CPS check) shall be on file for each staff within the first 30 days of employment. Staff 2 (date of employment 1/15/2024) did not have a completed search of the central registry on file. There was no documentation of having checked for the status of the CPS check by the 30th day of employment.

Standard 8VAC20-780-230-B
Individuals from independent contractors who do not meet staff qualifications
shall, when in the presence of children, be within sight and sound supervision of a
staff member. On 5/29/2024, an independent contractor was alone with 5 preschool children in the indoor gym. There were no center staff present. The independent contractor stated that she is often alone with the children.

Each grouping of children shall have at least one staff in each grouping of children who meets lead teacher qualifications There was no documentation to support that either of the two staff alone with 14 children meet lead teacher qualifications.

Standard 8VAC20-780-500-B
Repeat violation. The diapering surface shall be cleaned with soap and water and sanitized after each use. The surface of the item is sprayed or dipped into the disinfectant solution and allowed to air dry on the surface for a minimum of two minutes or according to the disinfectant solution instructions. A staff person in the infant room cleaned and sanitized the diaper changing table, but did not allow the disinfectant to dry for a period of at least 2 minutes.

Repeat violation. Medication shall be given to a child only with written authorization from the parent. One emergency medications for one child did not have written authorization from the parent.

Children three years of age or younger may not be offered foods that are considered to be potential choking hazards. In the twos room, a child was served hot dog rounds that were brought from home. Hot dog rounds are considered a choking hazard.
