An unannounced monitoring inspection was initiated on October 14, 2025 and completed on November 7, 2025 with a local agency in response to a self-report received by the licensing office on October 14, 2025 relating to supervision. The inspector reviewed 1 child?s record on site. This inspection included document review, a tour of the facility, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on December 8, 2025. A POC submitted after this date will not appear on the public website.
Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection and guidance. On 10/14/2025 at 10:44 a.m., Child #1 (age 3 years) was left alone on the playground. Staff 1 and Staff 2 were bringing a group of children inside from the playground. One of the children ran inside of the building, and one staff ran after the child. The second staff followed the other children into the building without properly conducting a name to face count of the children. At 11:28 a.m., Staff 1 went outside and found Child 1 alone on the playground. The child was alone for approximately 45 minutes.
Plan of Correction: We recognize that proper Name-to-Face transition procedures were not followed, resulting in a child remaining outside for an extended period. Please refer to the attached documents that reflect the changes that were made. This is unacceptable and deeply concerning to us as a school community. In response, we self-reported the incident, removed staff who failed to follow mandatory safety protocols, and immediately retrained all team members on transition procedures and active supervision. Additionally, we are adding a dedicated administrative staff member to oversee transitions and strengthen accountability. We remain fully committed to providing a safe, nurturing, and well-supervised environment for every child in our care.

Children under 10 years of age shall always be within actual sight and sound supervision of staff. On 10/14/2025, Child 1 (age 3 years) was left alone on the playground from 10:44 a.m. to 11:28 a.m. The child was alone for approximately 45 minutes.
Plan of Correction: We acknowledge that a child was left unsupervised, and we fully recognize the gravity of this lapse. This incident does not reflect our standards or our commitment to providing a safe learning environment. Upon discovery, we acted immediately terminating involved staff, conducting mandatory retraining, and implementing enhanced safety measures including more frequent supervision checks and reinforced transition protocols. We are committed to ensuring this type of incident never happens again and to upholding strict compliance with all supervision requirements. Please refer to the attached documents that reflect the changes that were made.

The operational responsibilities of the licensee shall include ensuring that the center's activities and services are maintained in compliance with the center's own policies.
Staff 1 and Staff 2 failed to follow the center's supervision policy. As a result, Child 1 (age 3 years) was left alone for 45 minutes. The staff handbook says "Name to face protocol is followed with attendance tracking periodically to ensure each teacher is aware of and visually locating the designated children in their class throughout the day."
Plan of Correction: We take this violation with the utmost seriousness. Immediately upon discovering that the center?s Name-to-Face protocol was not followed, we terminated the responsible staff members in accordance with our zero-tolerance safety policy. We self-reported the incident to licensing and CPS the same day and immediately retrained all remaining staff on supervision, threshold transitions, and Name-to-Face procedures. The training was a one-on-one meeting with teachers. Please see the attached form named `Employee Expectations?. We have strengthened internal monitoring by incorporating the attached 15-minute checks and are willing to submit the 90-day worth of documentation. We are also hiring a dedicated Operations Support Manager (Job description attached) whose primary responsibility will be to oversee transitions and ensure ongoing compliance with all safety policies. While we are in the process of hiring, the admin staff continues to make more frequent walkthroughs throughout the day. Our program remains committed to maintaining the highest standards of child supervision and accountability.
