Documentation of a completed search of the central registry (CPS check) shall be submitted prior to employment for each staff. Staff 1 (employed for 4 months) did not have documentation of a submitted search of the central registry on file.
Plan of Correction: The background check has been completed. We now have a dedicated hiring coordinator whose job is to ensure that all documentation is complete. The hiring coordinator is responsible to ensure that the attached hiring checklist is complete before the employee is able to start. The school admin staff is required to go through all employee files on a monthly basis to identify any paperwork / training that is about to be expired. This will allow us to be proactively stay in compliance.

The center is required to do the appropriate background checks prior to employment from any state in which the individual has resided in the preceding five years. The center did not request a search of the child abuse and neglect registry or a criminal record check from one previously resided state for Staff 4, who has been employed for 3 weeks, or for Staff 7, employed for 4 months. Each staff was working with children at the time of the inspection.

Plan of Correction: The authorization has been documented. The teachers are instructed to not accept any medication from parents without the authorization forms. The administration is required to check on all medications on a weekly basis and sign off.

Each center shall maintain and keep at the center a separate record for each child enrolled which contains required information. Child 3, Child 5, Child 6, and Child 7 did not have documentation of the name, address, and phone number for two emergency contacts.
