During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on March 26, 2026, and completed on March 26, 2026, as a part of the licensure period. The on-site inspection began at 11:30 am and ended at 12:30 pm. The inspector reviewed compliance in the areas listed above. There were four children present and one caregiver; the total point value was ten. The inspector reviewed four children?s records and two caregiver/household member(s) records on-site.
This inspection included document review, tour of the facility, interviews, and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-530-F
The family day home shall ensure no soft bedding of any kind shall be used under or around infants including, but not limited to, pillows, quilts, comforters, sheepskins, or stuffed toys. A boppy pillow was observed in the corner of the crib with child #1, seven months in age and who was asleep.
Plan of Correction: The provider removed the boppy pillow while the inspector was on cite. Per the provider: I will not put anything in the crib while infants are in the crib.
Standard 8VAC20-800-750-A
When topical skin products such as sunscreen, diaper ointment and lotion, oral teething medicine, and insect repellent are used, written parent authorization noting any known adverse reactions shall be obtained at least annually. 1. The provider stated oral teething medicine had been administered to child #1. There was no written parent authorization to administer the oral teething medicine. 2. The provider stated diaper cream had been administered to children when needed. There was no written parent authorizations to administer to children.
Plan of Correction: Per the provider: I will have parents complete the written authorization forms.
