An unannounced on-site self-report inspection was conducted on 8/1/2022. The inspector arrived at 10 AM and departed at 10:45 AM. A self-report was received in the licensing office on 7/26/2022. The preponderance of evidence of forbidden action was not supported and self-report is not valid.
