Inspector Notes

An unannounced non mandated monitoring inspection was conducted on 8/17/2022. There were 32 children, ages 2 month--3 years, and 10 staff members and the director present during the inspection. The inspector observed the following: outside play, snack, free choice activities, diaper changing, and story time. A total of 5 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: The inspectors arrived at 2:15 PM AM and departed at 4:30 PM.

Based on review of five staff's records, the center failed to ensure that all TB screenings were at the time of employment and prior to coming inconstant with children .

Evidence:
1. Staff # 2 was hired on 4/4/2022 and the TB screening/test was completed on 5/23/2022.
Plan of Correction: In the future all TB screenings/tests will be completed at the time of employment.
