Violations

1
Standard 8VAC20-780-270-A
Based on observation, the center failed to ensure that all areas and equipment are safe,
clean and operable condition.

Evidence

The resilient surfacing for both of the installed playground equipment had a measurable
separation between the ground and the resilient surfacing. Some areas could measure
up to 2 inches of a gap between that could pose a tripping hazard to the children in care.
Plan of Correction: The director will have the maintenance person to look at the resilient surfacing
and will make a place to fix the gap.
