Inspector Notes

An unannounced on-site monitoring inspection was conducted on 04/15/2024. There were 41 children, ages 2 ? months-5 years, and 14 staff members present during the inspection. The inspector observed the following: free choice activities outside feeding, floor plan for infants and snack. A total of 5 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: staff records. The inspector arrived at 9:30 AM and departed at 11:15 AM

Based on review of diaper ointments, the center failed to ensure that all over the counter skin products shall not be kept or used beyond the expiration date of the product.

Evidence:

Two expired diaper ointments (2/2024) were in the Butterflies room.
Plan of Correction: The site director discarded the diaper ointments during the inspection.
