Inspector Notes

An unannounced monitoring inspection was conducted on 4/22/24 from 10:30am to 11:40am with the provider who was present with two assistants. There were 11 children in care, ranging in age from six-months to four-years-old, for a total of 26 points. Children were observed playing outside, transitioning inside and washing hands, having diapers changed, participating in circle time, and having free play. Four child records, three caregiver records, one household member record, and one pet record were reviewed. The provider and one assistant have current certification in CPR and First Aid. The provider does not give medication and understands that they must obtain certification in Medication Administration Training (MAT) in order to give medication in the future. The emergency drill log, attendance, and menu were also reviewed. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Based on review of one household member record, the provider did not obtain a current tuberculosis (TB) screening for an adult household member every two years as required. Evidence: The most recent TB screening on record for Household Member A was dated 3/18/22.
Plan of Correction: Household Member A obtained a new screening on 4/23/24.
Standard 8VAC20-800-60-B
Based on review of four child records, the provider did not obtain all of the required documentation for child records. Evidence: The record for Child A did not contain documentation of the name, address, and phone number of a second emergency contact in case of emergency when a parent cannot be reached; and a parent?s signature authorizing the provider to obtain emergency medical care for the child. The record for Child D did not contain documentation of the address for an emergency contact or the name, address, and phone number of a second emergency contact.
