Inspector Notes
A complaint investigation was conducted on 5/15/2023 in response to a complaint received by the Fairfax Licensing Office on 5/11/2023 in regards to the improper transportation of children.
Interviews were conducted with the provider and records were reviewed.

Caregiver 1's record did not contain the results of a national criminal record check.
Evidence:
Caregiver 1's record did not contain a completed sworn statement or affirmation. Caregiver 1 provided care to children on 5/11/2023.
Evidence:
Caregiver 1's record did not contain the caregiver's job title, date of employment or volunteering, the results of required background checks, documentation of a tuberculosis screening, documentation of education and training, or the name address and telephone number of a person to contact in an emergency.
Evidence:
On 5/11/2023 Caregiver 1 was acting in the capacity as the substitute provider while the provider was absent from the family day home. There was no record to document Caregiver 1's arrival and departure from the role of substitute provider for 5/11/2023.
Evidence:
Caregiver 1's record did not contain a current Report of Tuberculosis Screening form published by the Virginia Department of Health or a form consistent with it documenting the absence of tuberculosis in a communicable form.
Evidence:
Caregiver 1's record did not contain documentation of having received an orientation.
Evidence:
1. On 5/11/2023 Caregiver 2 took Child 1 off premises in her personal vehicle to run a personal errand.
2. Child 1's record did not contain written permission from the child's parent for the provider to take the child off the premises of the family day home.
