During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated on September 9, 2025 and completed on September 9, 2025, as a part of the provisional licensure period. The on-site inspection began at 9:30am and ended at 10:00am. The inspector reviewed compliance in the areas listed above. There was 1 child present and 1 caregiver; the point total was 3. The inspector reviewed 3 children?s records and 2 caregiver/household member(s) records on-site and electronically on September 9, 2025.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 16, 2025. A POC submitted after this date will not appear on the public website.

Documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.

The findings of the most recent licensing inspection were not posted on the premises.

REPEAT VIOLATION
Children shall be protected from materials that could be swallowed or present a choking hazard.

A latex balloon was observed on the floor in the child care area.
Plan of Correction: It was corrected. THere will be no balloon on the floor.
Standard 8VAC20-800-320-B
Child-resistant protective covers larger than 1-1/4 inches in diameter shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age.

A plugged-in powerstrip and one outlet was observed without protective covering. The child in care during the inspection was aged 23 months.
Plan of Correction: The missing outlets were covered.

Pressure mounted gates shall not be used as protective barriers at stair openings.

A pressure mounted gate was observed at the opening of the stairs accessible in the child care area.
Plan of Correction: The mounted door will be replaced by a more protector.

Dogs shall be vaccinated for rabies.

The rabies vaccination for Dog 1 expired 71 days prior to the inspection.

The diapering surface shall be separate from areas used for children's activities.

A bouncy seat and items used by children were observed to be stored on the diapering surface,
