During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated on February 13, 2026 and completed on February 13, 2026 in response to a a complaint received by the licensing office on February 10, 2026 relating to allegations of improper transportation of a child. The inspector reviewed 2 children?s records and 1 staff record on-site and electronically on February 13, 2026. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on MArch 2, 2026. A POC submitted after this date will not appear on the public website.
Violations
3The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.
The findings from the most recent licensing inspection were not posted in the facility on the date of inspection.
The protective barrier at the bottom of the stairs was observed to be open.
General written permission shall be obtained from the parent of each child for the provider to take the child off the premises of the family day home.
On February 10, 2026 the provider took Child 1 off premises in their personal vehicle. The parents of Child 1 did not provide written permission for the provider to tak ethe child off premises. The provider did not inform the parents in advance of the trip.
Transportation authorization is now required at enrollment before the first day of care.
A compliance checklist has been added to each child?s file to ensure all required forms are completed.
