22.1 Religious Exempt
During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated and completed on 6/11/2025, in response to a complaint received by the licensing office on 6/6/2025, relating to allegations of staffing and supervision, physical plant, programs, and special care provisions and emergencies. The inspector was on-site from 10:20am until 1:10pm. The inspector reviewed 8 children?s records on-site. This inspection included document review, interviews, a tour of the facility, and observations.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on June 26, 2025. A POC submitted after this date will not appear on the public website.
Violations
41. In the two?s classroom, the youngest child was 22 months. There were 6 children observed on 6/11/2025 with one staff member exceeding the ratio by 1 child.
2. On several occasions, Staff B (3-Year-old classroom) when needing to step out of the classroom, would leave their children sitting outside of the classroom against the wall so Staff E in the neighboring classroom could watch both classrooms until Staff B returns. Leaving Staff E out of ratio. Staff E?s youngest child is 22 months.
1. On several occasions, Staff C (4-Year-old classroom) when needing to step out of the classroom, would leave their children in the classroom and ask the neighboring teacher, Staff B (3-Year-olds) to stand in between both classrooms leaving Staff B out of ratio.
2. On several occasions in the evening, Staff C (4-year-old classroom) and Staff F (School-age classroom) bring their children up to Staff B?s classroom for the end of the day. Staff B?s youngest child is 3-years old. Once all of the children merge, Staff B has had up to 18 children alone.
On 6/11/2025 in the infant classroom there were 8 children in care. Staff A was observed sleeping for approximately 2 to 3 minutes until awoken by management.
1. On 6/11/2025, there was no cushioning material observed under the playground equipment, several staff stated there has been no mulch in over 6 months under the playground equipment.
2. On the outdoor playground there was a playset that was cracked and broken.
