During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated and completed on 8/14/2025 in response to a complaint received by the licensing office on 8/13/2025 relating to allegations of Staff to child ratio. The inspector reviewed 3 staff records onsite. This inspection included document review, tour of the facility and interviews.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/3/2025. A POC submitted after this date will not appear on the public website.

REPEAT VIOLATION The required ratio for children ages zero to 16 months is one staff member per four children.

On 8/14/2025, interviews with staff and documentation review revealed that the infant room was out of ratio for approximately 15 min. The youngest child in care at the time was 15months. Staff A had 4 children in care until 6:30am when another child arrived bringing the total children in care to 5. Another child arrived at 6:35am bringing the children in care to 6. Another teacher arrived at 6:45am.

REPEAT VIOLATION The required ratio for children ages 24 months to 36 months is one staff member to 8 children.

On 8/14/2025, interviews with staff and documentation review revealed that the Three?s Classroom that is merged in the morning was out of ratio for approximately 2 minutes. The youngest child in care was 2 years old. Staff B had 8 children in care until 8:28am when the 9th child showed up. Another teacher arrived at 8:30am.
