Inspector Notes
Licensing Inspector Anderson conducted an Initial Subsidy Health and Safety Inspection on November 2, 2022. There were 10 children ages 3 months to 4 years old present under the direct care of 3 staff. The Licensing Inspector reviewed 3 children?s records and 3 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

1 Documentation of a negative tuberculosis screening for Staff #3 was not completed on/before the date of hire. Staff #3 hire date is 5/31/2022; tuberculosis screening completed on 9/16/2022.
2 Documentation of a negative tuberculosis screening for Staff #1 was unavailable.
EVIDENCE:
Unlocked hazardous cleaning agents were observed in the infant/toddler classroom.
EVIDENCE:
The warm water located in the sinks that are used for handwashing registered as 125 degrees F. The warm water temperature exceeded maximum allowable temperature.
