Inspector Notes

Licensing Inspector Anderson conducted an Initial Subsidy Health and Safety Inspection on November 2, 2022. There were 10 children ages 3 months to 4 years old present under the direct care of 3 staff. The Licensing Inspector reviewed 3 children?s records and 3 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the Vendor failed to have proof of identity and age for each child.

EVIDENCE:
Licensing Inspector reviewed 3 children records. All 3 records did not include documentation of proof of identity.
Plan of Correction: Director will request information from each parent. Documentation of findings will be maintained in each child's record.
Standard 8VAC20-790-560-A
Based on record review, the Vendor failed to have documentation of a tuberculosis skin test/screening submitted at the time of employment for each staff.

EVIDENCE:
Licensing Inspector reviewed 3 staff records.
1 Documentation of a negative tuberculosis screening for Staff #3 was not completed on/before the date of hire. Staff #3 hire date is 5/31/2022; tuberculosis screening completed on 9/16/2022.

2 Documentation of a negative tuberculosis screening for Staff #1 was unavailable.
Plan of Correction: Director will ensure each staff member completes a tuberculosis skin test or screening. Documentation of the findings will be maintained in each staff record.

Based on inspection, the Vendor failed to keep hazardous substance in a locked place using a safe locking method that prevents access by children.

EVIDENCE:
Unlocked hazardous cleaning agents were observed in the infant/toddler classroom.
Plan of Correction: Corrected during inspection. All hazardous substances will be stored in a locked cabinet.

Based on inspection, the Vendor failed to ensure sinks located near the toilets and that are supplied with running warm water does not exceed 120 degrees F.

EVIDENCE:
The warm water located in the sinks that are used for handwashing registered as 125 degrees F. The warm water temperature exceeded maximum allowable temperature.
Plan of Correction: Director will contact maintenance to adjust the warm water temperature not to exceed 120 degrees F.
