Inspector Notes

An unannounced inspection was initiated on 04/01/2025 and completed on 04/07/2025, in response to a complaint received by the licensing office on 03/26/2025 relating to allegations of Forbidden Actions. The inspector reviewed 1 child?s records and 2 staff records on-site. This inspection included: document review and interviews.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

The following actions or threats thereof are forbidden, restricting movement through binding or tying.

Child # 1 (16 months) was restrained by placing the child?s shirt over the back of the chair. The child was unable to freely exit the chair.
Plan of Correction: Staff stated that this happened one time, and it will not occur again.
