Inspector Notes

A monitoring inspection was conducted today between 7;30am and 9:15am. There were 26 children present with 6 staff supervising. The inspector investigated a self reported incident regarding safe sleep practices. The inspector also reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The children were observed arriving, playing and interacting with staff. A total of 4 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of 3 children's records, one did not contain a physical examination.
Evidence
Child #1, first day of attendance 6/21/22, does not have a physical examination in their record.
Plan of Correction: I will contact the parent and get the physical from signed by a doctor.
Standard 8VAC20-780-370-1
Based on photographic evidence, an infant was not placed in their crib on their back.
Evidence
Child #2 (4 months old), was placed in their crib on their stomach. Parent of child #2 was sent a photographh of the child sleeping in their crib on their stomach.
Plan of Correction: Staff was re-trained in safe sleep practices. Director will closely monitor the infant room to ensure compliance.
