Inspector Notes

A monitoring inspection was conducted today between 11:30am and 1:30pm. There were 32 children present, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The children were observed playing outside, playing with table toys, and circle time. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards and violations were documented on the violation notice issued. Standard 8VAC20-780-130-A
Based on review of 5 children's records, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child's first day of attendance.
Evidence
Child #2, first day of attendance 10/13/2023, does not have documentation in their record of receiving immunizations.
Plan of Correction: I will get the immunization record from the parent and make sure we always it before their first day.
Standard 8VAC20-780-130-E
Based on review of 5 children's records, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence
Child #2, under the age of two, does not have documentation of additional immunizations once every six months.
Plan of Correction: I will make sure to obtain an updated immunization record every six months for all children under the age of two.
Standard 8VAC20-780-500-A
Based on observation the center does not have a leakproof or plastic lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal of a diaper.
Evidence
The garbage can in the toddler bathroom used to dispose soiled diapers has a swing lid, that the staff members hand or soiled diaper will touch when disposal.
Plan of Correction: I will order a new garbage can today.
Standard 8VAC20-780-60-A
Based on review of 5 children's records, the center did not maintain a complete record for 2 children.
Evidence
The record for child #1 did not contain the that the original proof of identity had been reviewed prior to the child's first day of attendance.
The record for child #2 did not have documentation of an address for the second emergency contact.
Plan of Correction: I will make sure all children's records have all the required information and are complete.
