Inspector Notes

An investigation was initiated on May 23, 2024 regarding a complaint received by the Fairfax Licensing Office on May 22, 2024 alleging out of compliance with transportation, emergency drill, crib, physical plant, behavior guidance and staff qualification standards. Interviews were conducted, emergency records, staff records and policies and procedures were reviewed. The information gathered during this inspection supports the allegations that the center was out of compliance with transportation, emergency drill and crib standards. The complaint is deemed valid. Standard 8VAC20-780-440-L
Based on observation, a pillow is being used by a child under two years of age.
Evidence
A boppy pillow is being used by staff # 5 and #6 to assist child #1 (9 months old) in staying asleep when laying them in the crib.
Plan of Correction: No more Boppy pillows will be used to assist the baby during nap time.
Standard 8VAC20-780-550-G
Based on review of emergency documents, the emergency evacuation and shelter in place documentation is not accurate for the month of April.
Evidence
Documentation for the monthly emergency evacuation drill and shelter in place for the month of April recorded the number of participating children and staff incorrectly. Staff #1 confirmed the documentation was not correct as documented. The form recorded 32 children with 10 staff participating. The actual number was of children and staff participating was 6 and 2.
Plan of Correction: We will make sure we document all of our fire drills and shelter in place are documented correctly from now on.
Standard 8VAC20-780-580-B
Based on interview with staff, any vehicle used by the center for transportation of children did not meet all the requirements.
Evidence
Child #2 was transported by a staff member to a field trip in their personal vehicle. The center did not ensure the vehicle was insured with at least the minimum limits established by Virginia state statues and it was not confirmed the vehicle met the safety standards set by the Department of Motor Vehicles .
Plan of Correction: We will always make sure any vehicle used by staff to transport children is insured.
