Inspector Notes

An unannounced on-site monitoring inspection was conducted on 4/11/2024. There were 42 children, ages 1 years-5 years, and 11 staff members present during the inspection. The inspector observed the following: free choice activities, large muscle activity outside, circle time and lunch. A total of 5 children?s records and 5 staff records, were reviewed. The inspector arrived at 9:30 AM and departed at 11:15 AM.

Based on review of staff?s records, the center failed to ensure that all required out of state background checks were obtained.

Evidence:

Staff # 5 had a documented employment date of 11/28/2022. Staff # 5 indicated on the sworn statement that they have lived outside of the state of Virginia in the past five years. The state indicated on the sworn statement does not participates in the National Fingerprint File which requires a CPS and Criminal check from that state.

Standard 8VAC20-780-160-A
Based on interview and review of staff record, the center failed to ensure that all staff had a
negative tuberculosis (TB) screening at the time of employment, no more than 30 days
prior to hire, and before coming in contact with children.

Evidence:
1.Staff # 1 started employment on 02/26/2024 and the TB screening was dated 03/01/2024.
2.Staff # 3 started employment on 03/12/2024 and the TB screening was dated 03/13/2024.

Based on observation, the center failed to ensure that all diaper requirements were met.

Evidence:
1.The trash can in the Mixed Deliver classroom that held soiled diapers had a barrier that prevented the staff member?s hands nor soiled diaper from touching the exterior surface of the storage system during disposal.
