Inspector Notes

An unannounced monitoring inspection was initiated on 3/23/2026 and completed on 3/26/2026 in response to a self-report received by the licensing office on 3/20/2026 relating to administration. The inspector reviewed 1 child record and 2 staff records on-site. This inspection included document review and interviews.

Violations have been cited related to noncompliance resulting in an injury to a child while under the supervision of the facility/provider that required outside medical treatment. Please see the violation notice for more details.

Standard 8VAC20-780-80-B-2
The center shall inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.

Child #1 sustained an injury, while under the supervision of the center on 3/4/2026, that required outside medical attention. The center did not notify the department that the injury occurred until 3/20/2026.
