Inspector Notes

A monitoring inspection was initiated on August 17, 2023 and concluded on August 17, 2023. There were 48 children present, ranging in ages from 16 months to 11 years, with 14 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 15 staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard 8VAC20-780-240-A
Based on a review of staff records and interview, the center failed to ensure that staff complete the Virginia Department of Education sponsored orientation course within 90 calendar days of employment.

Evidence:
1. The record of staff 12 (DOH 03/06/2023) did not contain documentation of the Virginia Department of Education sponsored orientation.
2. Staff 2 acknowledged it was not completed.
Plan of Correction: I will make sure that all the staff complete their 10 hour orientation within the first 90 days of employment.
Standard 8VAC20-780-280-B
Based on observation and interview, the center failed to ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In the one-year old room, there was shelf labeled Lysol. The shelf contained Lysol spray. The label on the Lysol stated, "Flammable" and "keep out of reach of children."
2. Staff 2 confirmed that the Lysol was not kept in a locked place using a safe locking method that prevents access by children.
Plan of Correction: I will remind the teachers during our staff workday to lock up the Lysol in the closet or in the locked cabinet above the sink.
