8VAC20-780 Staffing and Supervision.

Inspector Notes

An unannounced monitoring inspection was initiated on 04/14/2025 and completed on 04/18/2025 in response to a self-report received by the licensing office on 03/13/2025 relating to staffing and supervision. This inspection included: document review and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 04/25/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-F
Children under 10 years of age always shall be within actual sight and sound supervision of staff.

On March 13, 2025, Staff 2 left Child 1 (2 years old) in Classroom #1 for under a minute, while taking the group of children outside. Staff 1 found Child 1 in Classroom #1 crying and trying to open the door to go outside.

Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall ensure that all the center's own policies and procedures that are required by the standards. The standards require procedures for identifying where children are at all times. The center's policy training manual states,"When staff are supervising children, they must ensure their care, protection, and guidance. Children are always within sight and sound of a supervising adult."

On March 13, 202, less than a minute passed when staff and children walked outside to the playground. Staff 1 was walking down the hall to another classroom and passed by Classroom #1 and saw a child by the back door crying and trying to open the door. Staff 1 walked in to help the child and noticed there was not a teacher inside the classroom.
