Inspector Notes

A monitoring inspection was conducted on 09/06/2022. There were a total of 13 children with 2 staff members present. Children were observed finishing lunch, reading a book with the teacher, and playing with blocks. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 1 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-160-A
Based on a review of staff records, each staff member did not submit documentation of a
negative tuberculosis screening completed within the last 30 calendar days of the date of
employment.
Evidence:
Staff #1 (date of hire 8/29/2022) had a tuberculosis test dated 6/15/2022.
Plan of Correction: We will make sure all employees have their TB test or screening within the 30 days of employment.
Standard 8VAC20-780-60-A
Based on a review of child files, the center did not keep a record of place of employment for each parent.
Evidence:
Child #1 did not have a place of employment listed for their parents.
Plan of Correction: I will reach out to the parents to get their place of work. I will also audit all of the files to make sure all the children have it.
