Inspector Notes

Licensing Inspector conducted an unannounced Renewal Inspection on February 9, 2023. Upon arrival, there were 19 children ages 2-4 years old under the care of 7 staff. The Licensing Inspector reviewed 4 children?s records and 4 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the Licensee failed to complete out-of-state Background Checks for staff members who have lived out of the State of Virginia within 5 years prior to the date of hire.

EVIDE3NCE:
Licensing Inspector reviewed 4 staff records.

Staff #4 indicated on their Sworn Statement that they had in fact lived out side of the State of Virginia within 5 years prior to the date of hire (10/13/2022). Required Background Checks ((tate of Alabama Interstate Child Abuse & Neglect & State of Alabama Interstate Sex Offender Registry Check) for Staff #4 were not completed.

Staff #4 indicated on their Sworn Statement that they had in fact lived outside of the State of Virginia within 5 years prior to the date of hire (10/13/2022). Required Background Checks (State of Georgia Interstate Child Abuse & Neglect, State of Georgia Interstate Sex Offender Registry Check & Interstate CR Background Check) for Staff #4 were not completed.
Plan of Correction: Center will submit documentation to the licensing inspector have been completed.
