Licensing Inspector Anderson conducted an unannounced complaint inspection in response to allegations received by the licensing office on May 22, 2024, in the areas of adult to child ratios. The investigation of the complaint was initiated on May 24, 2024 and findings were reviewed on
June 26, 2024.

Based on record review and interviews, the Center failed to inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.

EVIDENCE:
On Wednesday, December 6, 2023 at approximately 12:50 pm, Child #1 fingers were caught in the classroom door. Child #1 received outside medical care was received on 12/06/2023. The Center did not submit a Injury Requiring Outside Medical Attention (IROMPT) report.
