During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.
Inspector Notes
An unannounced, on-site renewal inspection was initiated on 01/27/2025 and completed on 02/03/2025. The on-site inspection began at 10:30 AM and ended at 12:30 PM. The inspector reviewed compliance in the areas listed above. There were 31 children present and 8 staff. The inspector reviewed 5 children?s records, and 5 staff on-site. This inspection included document review, tour of the facility and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 02/10/2025. A POC submitted after this date will not appear on the public website.
Violations
9Staff #2, who had been employed for six months, before the central registry search was returned.
Staff # 4 did not have the required 24 hours of training to qualify for program leader.
Staff #5 has been employed over 90 days and has not taken the course.
In the bathroom used by the children, the cleaning solution and sanitizing solution were in an unlocked.
The center did not have an injury prevention plan.
The center did not have a playground safety plan.
Evacuation drills for November and December of 2024 were not conducted.
Sanitizing wipes were used to before and after lunch.
The record for Child #3, enrolled for 2 years, was missing the following required components: address for one of the required designated people to call in an emergency if a parent cannot be reached.
The record for Child #5, enrolled for 3 months, was missing the following required components: addresses for both of the required designated people to call in an emergency if a parent cannot be reached.
