During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/17/2024 and completed on 10/17/2024. The on-site inspection began at 10:05 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 1 caregiver; the point total was 12. The inspector reviewed 5 children?s records and 4 caregiver/household members records.
This inspection included document review, tour of the facility, interviews, observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Repeat violation:
Household members 14 years of age and older must obtain the results of a central registry search by the end of the 30th day of residence.
Household member # 3 did not have central registry results during the inspection and has been residing in the home for over 90 days.
Plan of Correction: The provider will resubmit the central registry search.
Standard 8VAC20-800-60-B
Children's records are required to contain parental name, address and phone number for place of employment, name, address and phone number for child's physician, 2 persons to contact in case of an emergency, name and policy number of child's medical insurance, first day of attendance, parent's signed acknowledgement, proof of child's age and identity, proof of insurance liability.
The record for child #1 was missing the above information. The child was present during the inspection and had been in care for 2 days.
