During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was completed on 10/14/2025, as a part of the licensure period. The on-site inspection began at 10:50 am and ended at 11:45 am. The inspector reviewed compliance in the areas listed above. There were (2) children present and (1) caregivers; the point total was (6). The inspector reviewed (3) children?s records and (1) caregiver/household members records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1There was no documentation of immunizations for child #3, who was not in care during the inspection and had been in enrolled for one month.
