During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 10/7/2024. The on-site inspection began at 9:51 am and ended at 1:15 pm. The inspector reviewed compliance in the areas listed above. There were 119 children present and 27 staff. The inspector reviewed 10 children?s records and 10 staff records on site. This inspection included document review, a tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/14/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-140-A
REPEAT VIOLATION
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no documentation of a physical examination in the record of child #2 , who had been enrolled for one month and 11 days.
Plan of Correction: Instead of an all hands on deck approach, director has taken responsibility of ensuring children's files are complete before start date, and will have a second set of eyes once completed.
Standard 8VAC20-780-520-C
If diaper ointment or cream is used, the following requirements shall be met: written parent authorization noting any known adverse reactions shall be obtained and the products shall be in the original container and labeled with the child's name.

In a toddler classroom, staff applied diaper ointment to two children without written parent authorization. One of the diaper ointment containers was not labeled with the child's name.
Plan of Correction: Management created new authorization forms/binders for each classroom to ensure adverse reactions lines are added and all lines filled out and writing on authorization form is legible. Management will audit authorization binders in each classroom weekly.

The center shall implement a monthly practice evacuation drill.

An evacuation drill was not conducted in September of 2024.
Plan of Correction: Office has created a new chart of responsibilities so there is someone clearly in charge of completing fire drills. Assistant director in charge of drills and has added dates to calendar.
Standard 8VAC20-780-570-E
Prepared infant formula shall be refrigerated, dated and labeled with the child's name.

A total of seven undated bottles of prepared infant formula belonging to three children were observed in the refrigerator in the infant classroom.
Plan of Correction: Classroom has implemented new process where closing teacher will create name and date label stickers for bottles/food for the next day and will leave for the opening teacher to apply to each food item.
