Inspector Notes

An unannounced monitoring inspection was conducted on 5/27/2025 in response to a self-report received by the licensing office on 5/8/2025 relating to supervision. The inspector reviewed 1 child?s record and 3 staff records on-site. This inspection included document review, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 6/3/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-F
Children under 10 years of age shall always be within actual sight and sound supervision of staff.

Child #1, age 2, left the classroom and entered the hallway where the child was unsupervised for approximately 1-2 minutes.
Plan of Correction: Office staff will monitor and ensure that name to face is boing completed at every threshold.
Standard 8VAC20-780-40-E
The center shall ensure their activities and services are maintained in compliance with their own policies and procedures that are required by these standards.

The center's supervision policy requires that children must be supervised by sight and sound at all times. At no time is a teacher to leave a child unsupervised. Child #1 was unsupervised in the hallway on 5/8/2025 for approximately 1-2 minutes.
Plan of Correction: Director will re-train staff on supervision policy as well as name to face during staff meeting.
