During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was conducted on 8/12/2025. The on-site inspection began at 10:30 am and ended at 1:45 pm. The inspector reviewed compliance in the areas listed above. There were 125 children present and 29 staff. The inspector reviewed 10 children?s records and 13 staff records on-site. This inspection included document review, a tour of the facility, interviews, observations and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 8/19/2025. A POC submitted after this date will not appear on the public website.
Violations
4In a two year old classroom, there were three children on rest mats during nap time that were spaced less than 12 inches from other occupied mats.
Staff #8 had a current CPR certification that was obtained online and did not include the hands on competency demonstration. Administration stated that staff #8 has supervised a group of children independently in the afternoon.

Child #4 and child #5 had injury reports that were missing any future action to prevent recurrence of the injury.
The record of staff #11, employed as a program leader over three months ago, was missing documentation to demonstrate that the individual possessed the education required by the job position.
