Inspector Notes
An unannounced monitoring inspection was conducted on 10/20/2025 in response to a self-report received by the licensing office on 10/07/2025 relating to supervision. The inspector reviewed 1 child?s record and 1 staff record on-site. This inspection included a partial tour of the facility, document review, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/27/2025. A POC submitted after this date will not appear on the public website.
Violations
2Children under 10 years of age shall always be within actual sight and sound supervision of
staff.
On 10/07/2025, Child #1, age 2, was left unsupervised on the playground for less than one minute during a transition from the playground to the classroom.
The center shall ensure their activities and services are maintained in compliance with
their own policies and procedures that are required by these standards.
The center's supervision policy requires that children must be supervised by sight and
sound at all times. At no time is a teacher to leave a child unsupervised. Name to face counts must be verified each time children cross a threshold. On 10/07/2025, Staff #1 gathered seven children to transition indoors from the playground. Two children were permitted to retrieve their water bottles. The door closed before Child #1 was able to enter the classroom. Child #1 stood outside of the classroom door for approximately 50 seconds before Staff #1 opened the door to retrieve the child.
