Inspector Notes

A mandated monitoring inspection was conducted on site at a field trip location on May 2, 2024 from 10:00 A.M.-11:15 A.M. and at the facility from 12:00 P.M.-12:30 P.M., the inspection concluded on May 6, 2024. There were seven children present, ranging in ages from three to five, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of the allergy list, the center failed to ensure that the list shall be dated.

Evidence:
The allergy list on site was not dated.
Plan of Correction: The last day of the current session was 5/2/2024. When the summer session begins the allergy list will be updated and the date will be listed on the document whenever it is updated.
