During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated on 01/23/2025 and completed on 01/24/2025, as a part of the licensure period. The on-site inspection began at 9:40am and ended at 12:00pm. The inspector reviewed compliance in the areas listed above. There were 60 children present and nine staff. The inspector reviewed five children?s records and five staff records on-site. Additionally documentation was reviewed electronically on 01/24/2025. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
3Staff #4, employed for more than two years, did not have documentation of requesting a central registry check or a criminal history check from a state they resided in the last five years.
Child #2 (19-month-old), who has been in attendance for more than 14 months, does not have documentation of additional immunizations once every six months.
The TB in the record of Staff #1 was completed more than three months after the date of employment and the TB in the record of Staff #2 was dated three months prior to the date of employment.
