During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 01/23/2025 and completed on 01/24/2025, as a part of the licensure period. The on-site inspection began at 9:40am and ended at 12:00pm. The inspector reviewed compliance in the areas listed above. There were 60 children present and nine staff. The inspector reviewed five children?s records and five staff records on-site. Additionally documentation was reviewed electronically on 01/24/2025. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

Staff #4, employed for more than two years, did not have documentation of requesting a central registry check or a criminal history check from a state they resided in the last five years.
Plan of Correction: The missing out-of-state background checks will be requested for Staff #4.
Standard 8VAC20-780-130-E
(Repeat Violation) The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.

Child #2 (19-month-old), who has been in attendance for more than 14 months, does not have documentation of additional immunizations once every six months.
Plan of Correction: Requested and obtained by 01/24/25.
Standard 8VAC20-780-160-A
(Repeat Violation) Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

The TB in the record of Staff #1 was completed more than three months after the date of employment and the TB in the record of Staff #2 was dated three months prior to the date of employment.
Plan of Correction: In the future, the center will not accept a TB that is outside of the required timeframes.
