Inspector Notes
A subsidy inspection was initiated on July 29, 2024 and concluded on July 29, 2024. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation and interview, the vendor did not ensure that the department?s representative was informed as soon as practicable, but no more than two business days of any injury to a child while under the vendor?s
supervision when a referral is made for treatment from a medical professional.
Evidence:
1. The vendor did not inform the licensing inspector of an injury, in which child #3 sought medical treatment for an injury that occurred while in care at the family day home.
2. Staff #1 confirmed that they did not inform the licensing inspector of the injury that was sustained by child #3 at the family day home, which resulted in requiring medical treatment.
