Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 11/20/2024. The on-site inspection began at 12:00pm and ended at 1:20pm. The inspector reviewed compliance in the areas listed above. There were 14 children present and 3 staff. The inspector reviewed 5 children?s records and 5 staff records on-site.
This inspection included document review, tour of the facility, interviews, observation.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
11. The record for Child #1, enrolled for six months, was missing the name, home address, and home phone number of one parent.
2. The record for Child #2, enrolled for 7 months, was missing one name, address, and phone number for 1 person to call in an emergency if a parent cannot be reached.
3. The record for Child #3, enrolled for 5 months, was missing both parents work phone number and place of employment.
