Inspector Notes

An unannounced Code Compliance inspection was conducted with the Director on 2/8/2024 from 9:45am through 12:05pm. There were 18 children present supervised by 5 staff. The children were observed singing and dancing, learning about black history month, learning shapes, and coloring rainbow crafts. Five child records were reviewed for documentation of immunizations of proof of age/identity verification. Four staff records were reviewed for background checks, annual health statements, and training. The program has multiple staff with current certification in CPR and First Aid, Daily Health Observation training, and Medication Administration Training. Areas of non-compliance are identified on the Violation Notice.

Based on staff interview, the center did not ensure there was a person on site trained to perform daily simple health screening and exclusion of sick children.

Evidence: There was no one present at the time of inspection who has training in conducting daily simple health screening.

Based on record review, the center did not obtain the results of an out of state criminal history background check prior to hire or request a search of the child abuse and neglect central registry within 30 days of employment for all states in which staff have lived within the previous 5 years.

Evidence: Staff #3 (Date of Hire: 8/30/2023) has lived in the states of Rhode Island and North Carolina within the last 5 years. Staff #3 did not have a Rhode Island criminal history background check on file. Staff #3's record was also missing child abuse and neglect central registry search requests for North Carolina and Rhode Island.
